A bulging black lump means skin invasion of the varicosity. The skin has become so thin that the underlying dark venous blood can be seen through the skin. The varicosity may itch, causing the patient to scratch and prone to rupture. Once rupture, there could be massive bleeding and the patient may syncope in the extreme situation...

Spider veins and reticular veins (blue-green veins) are very common. They are one type of the so-called chronic venous insufficiency. As time goes by, these lesions progress. They may increase in number or gradually bulge from the skin. Evidences from a medical research have shown that failure of microvenous valves in these small superficial veins is a key to the skin changes of venous insufficiency.

The doctors of CVS CLINIC joined the 20th Eurpean Venous Forum at Zurich University, Switzerland on June 27th~29th. European Venous Forum is one of the most important medical conference in the venous treatment field. At the conference, we discussed with the vein experts from several countries. We brought back the state-of-the-art conceptions and techniques, hoping to enhance the level of venous treatment in Taiwan.

To enhance the quality of medical service, the CVS CLINIC-Taichung cooperates with "Taiwan Philips Company" to upgrade the present ultrasound system in several aspects. Our new ultrasound system will not only facilitate the diagnosis of peripheral vascular disease but is also useful in the detection of pelvic or abdominal deep vein lesions. 
We thank the engineering team of Philips co. to set up the "cloud-based PACS-medical image system" for our clinic. The new ultrasound system can automatically transmit the sonographic images to the PACS system via intranet wifi.

The two sisters from Hong Kong who were treated for their varicose veins of legs and facial veins two years ago came back to CVS CLINIC for follow-up recently (Figure 1). One patient who was afflicted with varicose veins and venous eczema over her left calf due to severe reflux of the left great saphenous vein underwent endovenous laser surgery (EVLA) and micro-sclerotherapy two years ago. The clinical follow-up recently showed complete resolution of the venous eczema and improvement of the spider veins (Figure 2).

Our clinic developed a high-end painless sclerotherapy technique that greatly reduce the pain from injection. The needle-puncture pain, soreness and swelling sensation during sclerotherapy is reduced by approximately 70-100% with the new technique.

The technique has the following advantages: 

1. Reduce more than 70% of the pain. For example, if the the original pain scale on needle injection is 10, the pain is reduced to below 3 after using this technique.

2. Minimally-invasive or non-invasive to your body. 

3. Nearly no recovery period. The patient can walk and return to normal life almost immediately after the procedure. 

4. The technique does not interfere the efficacy of the treatment (sclerotherapy) per se.

Thanks to the patient's trust and recommendation to our clinic, we have treated several patients with difficult varicose veins in the first month since the opening of our Kaohsiung branch. These included patients with severely tortuous varicose veins, spider veins and recurrent venous ulcers treated in other hospitals before, and those from other country. After 4-hr operation, we successfully resolved a very difficult medical condition -- chronic deep vein thrombosis of lower limb. By using many types of wires, catheters, balloon, intravascular ultrasound (IVUS), stent, we opened up the thrombotic occlusion of calf veins, popliteal veins, femoral veins and iliac veins. The venous flow returned swiftly and the venous hypertension improved rapidly after the procedure. After one-night rest, the patient is able to walk freely now.

The paper title is  "Endovenous laser treatment of incompetent perforator veins- does the ablation method matter?". Treatment of the incompetent perforator vein (IPV) is more difficult than that of superficial vein. Two conclusions were drawn fro the present study: 1. With suitable indications, endovenous laser treatment of IPV significantly improved clinical condition 2. Direct ablation is more difficult in technique but has lower recurrence rate than indirect ablation.
